Every year, around the second week of the rains, I start seeing the same pattern in my OPD at Alright Hospital. Patients walk in complaining of fatigue, mild fever, or lower back pain, and almost all of them say some version of the same thing: “But doctor, it’s raining, how can I be dehydrated?”
That single assumption is exactly what makes monsoon dehydration so dangerous. Most people associate dehydration with summer, sweat, and scorching heat. The moment the temperature drops and the sky turns grey, they stop worrying about water intake. Your kidneys, however, do not get that memo. They continue to depend on adequate fluid intake every single day of the year, and the monsoon season quietly creates the perfect conditions for them to fall short.
If you live in Nagpur, understanding this connection between dehydration and kidney health in monsoon is not just useful information. It could be the difference between a mild, forgettable inconvenience and a hospital admission.
Why the Monsoon Is a Hidden Dehydration Trap
This is the part I spend the most time explaining to patients, because the logic feels backwards at first.
During the monsoon, humidity levels stay high even when the temperature is lower than peak summer. Your body still loses water steadily through sweating, but because that sweat evaporates more slowly in humidity, you don’t feel the dampness on your skin the way you do in dry heat. The result is that you lose water without the usual physical cue that tells you to drink more of it.
At the same time, the drop in temperature genuinely reduces your thirst signal. Thirst is regulated by hormonal and neurological triggers tied closely to heat and sweating. When the weather cools, that trigger weakens considerably, even though your body’s actual fluid requirement has not dropped nearly as much. Consequently that monsoon season also brings a spike in Stomach related infections, viral fevers, and diarrhoeal illness from contaminated water and food, and you have several separate mechanisms all pulling in the same direction, quietly pulling fluid out of your system.
A Lucknow-based nephrologist summed this up well in a recent interview, noting that dehydration during the monsoon usually goes unnoticed and only becomes obvious once it has already started affecting a person’s health. [1] That delay between cause and visible effect is precisely what makes this season’s dehydration harder to catch than summer dehydration, where thirst and sweat serve as early, obvious warnings.
What Dehydration Actually Does to Your Kidneys
Your kidneys are essentially sophisticated filtration units. Every day, they process roughly 150 to 180 litres of blood, filtering out waste products, balancing electrolytes, and regulating your body’s fluid volume. To do this efficiently, they rely on a steady, adequate flow of blood.
When you are dehydrated, your total blood volume drops. Your body responds by constricting blood vessels and reducing blood flow to organs it considers less immediately essential for survival, and the kidneys are one of the first to feel this reduction. Lower blood flow into the kidneys means a lower filtration rate, which means waste products such as urea and creatinine start to build up in your bloodstream instead of being cleared out.
If this state continues for long enough, or if the drop in blood flow is severe enough, it can progress into what we call pre-renal acute kidney injury, a sudden decline in kidney function caused by inadequate blood supply rather than direct damage to kidney tissue. Clinical research from Indian nephrology literature has repeatedly identified dehydration, alongside infections such as malaria, leptospirosis, dengue, and acute gastroenteritis, as leading contributors to the surge in acute kidney injury cases seen specifically during the monsoon months. [2] A multicentre Indian registry study similarly found that dehydration linked to acute gastroenteritis is a recurring and significant cause of community-acquired acute kidney injury across the country. [3]
The reassuring part is that pre-renal acute kidney injury caught early is usually reversible with prompt rehydration. The concerning part is that most people never realise it is happening until symptoms are already advanced, because early kidney stress rarely produces pain or any dramatic warning sign.
The Risk You Don’t Expect: Kidney Stones and Urinary Infections
When people think of kidney stones, they usually think about summer, not the rainy season. In my practice, I see a real spike in stone-related consultations during and immediately after monsoon, and the mechanism is straightforward.
When you drink less water than your body needs, your urine becomes more concentrated. Minerals such as calcium, oxalate, and uric acid, which would normally stay dissolved in a well-diluted urine stream, begin sticking together and crystallising instead. Over days or weeks, these crystals can grow into stones. Urologists have specifically flagged this seasonal pattern, explaining that when weather changes reduce thirst signals even though the body’s hydration needs remain unchanged, urine concentration rises and mineral crystallisation becomes far more likely. [4]
Concentrated urine also creates a more favourable environment for bacteria to multiply, which is a major reason urinary tract infections rise sharply during the monsoon. Reduced fluid intake means less frequent urination, and less frequent urination means bacteria have more time to establish themselves in the urinary tract before being flushed out. A recent report on seasonal kidney stone risk noted that this “hidden dehydration” pattern is now being seen even among people who spend most of their day indoors in air-conditioned or climate-controlled spaces, not just those working outdoors. Why This Matters More in India — And More in Nagpur
The stakes here are higher than many people realise. Chronic kidney disease already affects an estimated 138 million people in India, the second-highest caseload of any country in the world, and prevalence among Indian adults has climbed from roughly 11 percent in the 2011–2017 period to over 16 percent in 2018–2023. [6] A large national analysis using Global Burden of Disease data has similarly confirmed that chronic kidney disease now ranks among the leading causes of death in the country, with the burden continuing to widen across most states. [7]
This matters directly for hydration behaviour, because a kidney that is already working with reduced reserve — whether from undiagnosed early CKD, diabetes, or hypertension — has far less capacity to absorb the stress of repeated dehydration episodes. What might be a minor, self-correcting dip in kidney function for a healthy 30-year-old can be a genuinely dangerous drop for someone with even mild pre-existing kidney impairment, and in India, a significant share of the adult population falls into that category without knowing it.
For Nagpur specifically, our monsoon pattern of intense but irregular rainfall, high humidity between spells, and seasonal spikes in waterborne and vector-borne illness creates exactly the conditions I described above. I routinely see this combination in patients from across the city and from our Jafar Nagar and Lashkaribagh catchment areas alike.
Who Should Be Extra Careful
Certain groups face meaningfully higher risk during this season, and I ask my patients in these categories to be particularly vigilant:
- People with existing chronic kidney disease or a single functioning kidney
- People with diabetes or high blood pressure
- Older adults, whose thirst sensation naturally weakens with age
- Children, who dehydrate faster relative to their body size and often cannot communicate early symptoms clearly
- Anyone recovering from a recent infection, fever, or gastrointestinal illness
- People who work outdoors or travel extensively during commutes in the rain
- Office-goers who unconsciously replace plain water with tea, coffee, or sugary drinks through the day
Warning Signs You Should Never Brush Aside
Because monsoon dehydration develops quietly, the early warning signs are often subtle and easy to dismiss as “just the weather” or seasonal tiredness. Please take these seriously:
- Urine that is noticeably darker than pale yellow, or reduced in volume
- Persistent fatigue or lightheadedness that doesn’t improve with rest
- Dry mouth, dry skin, or a headache that lingers through the day
- Muscle cramps, particularly in the legs
- Lower back or flank pain, which can indicate either dehydration-related kidney stress or early stone formation
- Burning sensation while urinating, or a frequent urge to urinate with little output — both suggestive of a urinary tract infection
- In more advanced cases, swelling around the ankles, confusion, or a marked drop in urine output, which need immediate medical evaluation

Practical Hydration Steps for This Monsoon
The good news is that preventing monsoon-related kidney stress is genuinely simple, even if it requires a small shift in habit.
Drink on a schedule, not on thirst. Since your thirst signal is unreliable in this season, aim for roughly 2.5 to 3 litres of fluid a day for most healthy adults, spread evenly rather than consumed in large amounts at once. If you have existing kidney disease, heart failure, or are on fluid-restricted medication, please follow your doctor’s specific fluid targets instead of this general guideline.
Check your urine colour. Pale yellow is the target. Darker urine is one of the simplest, most reliable signs that you need more fluids right now.
Choose hydrating fluids over dehydrating ones. Plain water, coconut water, buttermilk, and oral rehydration solutions during illness all support hydration well. Excess tea, coffee, and sugary aerated drinks work against you, since caffeine has a mild diuretic effect that increases fluid loss through urine.
Be extra cautious during any bout of fever, vomiting, or loose motions. These are extremely common in the monsoon and cause rapid fluid loss. Increase fluid intake immediately at the first sign of illness rather than waiting for symptoms to worsen.
Don’t skip water because it’s raining. This sounds obvious written down, but it is the single most common mistake I see. Cooler weather does not reduce your body’s fluid requirement in any meaningful way.
Watch salt intake. High-salt monsoon favourites like fried snacks and pickles increase calcium excretion in urine, compounding the stone-formation risk that concentrated urine already creates.
